Historical Timeline/River of Life Develop timeline with partners: Timeline of partnerships to include relevant community history and events, key partnership events, key people, key processes.
How and where did this partnership start? What was important in its formation? What were key moments or events in the partnership? What other local, national, or international forces affected local development of the partnership?
CBPR Model Self-evaluation Guide Use CBPR Model Focus Group Guide: Give overview of meaning of each dimension of the Model (oval) Identify: –What’s most important to the partnership within each oval? –How has this construct or issue impacted our ability to work together (or strategize)? After going over existing constructs, ask: –What’s potentially missing within each oval? For each identified issue or construct, can ask:
1) Where did we start with this issue?
2) Where are we now?
3) Where do we want to be in 1-5 years?
4) How can we get there?
Visioning Choose the future date for a visioning process (1-2 years or 3-5 years from now) Questions to ask: What are the visions for sustainability that you have for the partnership? What would you like your partnership to look like 1-2 years from now, 3-5 years from now?
Report Updating Each month or quarter, update reports with the most recent data; visualize results
Mixed Methods Data Collection (This can be done sequentially or parallel timing)

QUANTITATIVE:

Key Informant Survey (KIS) and Community Engagement Survey (CES): Choose scales or questions that align with
constructs or issue from each dimension of CBPR model. If necessary, seek or create additional scales or survey questions.

QUALITATIVE:

Qualitative Interview and Focus Group Guides: Adapt questions to reflect targeted interests of partnership.

Data Analysis Compared Against National Benchmarks of Emerging Best Practices Analyze data and provide personalized partnership report of survey and qualitative analysis to partners. Compare partnership responses to national benchmarks of emerging best practices. Loop back to collective reflection on action steps. –What are our strengths that we want to maintain? –What are our challenges? –What do we want to do to strengthen our collaborative work ?
